Discharge Information
Your doctor will determine when you
are ready to be discharged from the hospital. Your
nurse will coordinate your discharge with the
physician and any other appropriate healthcare
providers. You may want to make arrangements with a
friend or family member to help you when it is time
to go home.
Your doctor and nurse will give you
instructions about post-hospital care.
If you have questions about your
diet, activities, medications or other matters,
please be sure to ask at this time. Remember, it is
OK to ask questions as it helps us provide the best
care possible. There are occasions when patients
need additional treatment or care after they leave
the hospital. There are a number of services
available in the community, such as home health
care, medical equipment, and personal emergency
response to assist you once you leave the hospital.
