

healthCHECKis offered for individuals who desire to play an active role in their own health.
The same testing that has traditionally been available at the health fair is available on Tuesday and Thursday mornings from 7 -11 am in the Lab on 1st Floor of the hospital or anytime during business hours at the CCMH Walk-in Clinic. Call 688.1222 with healthCHECK specific questions.
Results are mailed directly to the customer with a copy included for your healthcare provider. Fasting for 12-14 hours is required for accurate test results (water is permitted). Call 688.1222 for detailed information on healthCHECK testing.

A few important tips:
- Remember to fast for 12 hours if you are having the Wellness Profile.
- Always drink plenty of water before any blood work.
- healthCHECK tests are for screening purposes only and will not provide a diagnosis.
- CCMH will not file with your insurance company.
- Your health insurance may not reimburse you for these screening tests.
- Two copies of your results will be mailed to you within 3-5 business days.
- It is your responsibility to provide a copy of the results to your physician.

(Measures a protein produced by the male prostate gland and recommended by the American Cancer Society for men over age 50, or those with a family history of prostate cancer.)
hsCRP................................................. $40
(In conjunction with cholesterol levels, hsCRP is a strong predictor of peripheral artery disease. Cardio CRP may also help identify patients at risk of a first heart attack even with low to moderate lipid levels.
