This benefits guide highlights the benefit options available to you and explains how to enroll in your benefit options.

Please take a moment to review the list of benefits Campbell County Memorial Hospital (CCMH) offers and decide which options best suit the needs of you and your family. Then read through the detailed description of those plans listed in this guide. All benefits, except Life Insurance and Long Term Disability, are offered to Full-time and Part-time Employees. Long Term Disability and Life Insurance are offered to Full-time employees only. PRN (on-call) employees are eligible to participate in the Retirement program.

Dental Insurance CCMH and Employee 1st of the month following 30 days of employment
Vision Insurance *CCMH and Employee 1st of the month following 30 days of employment
Flexible Spending Employee 1st of the month following 30 days of employment
Basic Life Insurance CCMH 1st of the month following 30 days of employment
Supplemental Life Ins Employee 1st of the month following 30 days of employment
Long Term Disability CCMH 1st of the month following 30 days of employment
Paid Time Off (PTO) Accrued by Employee Immediately upon accrual
Personal Sick Leave (PSL) Accrued by Employee Immediately upon accrual
403(b) Tax Deferred Annuity Employee with CCMH Match
Employer match after 1 year of employment
Employee Immediately
457 (b) Deferred Compensation Plan Employee 1 month after enrollment

*Note:  The employer (CCMH) does not pay any of the premium cost for part-time employees
