Long Term Disability

What is Long Term Disability Insurance?

CCMH provides, at no cost to you, Long Term Disability Insurance. Long Term Disability Insurance pays you a portion of your earnings if you cannot work because of a disabling illness or injury.

Am I Eligible?

You are eligible if you are an active full-time employee.

When Can I Enroll?

You are automatically enrolled in the LTD Plan.

How Much Coverage Would I Have?

Long Term Disability Insurance pays you a benefit of up to 60% of your salary up to a maximum of $10,000 per month.

When is it Effective?

It is effective the first of the month following 30 days of employment. (Example, hire date is 9/15/10, insurance will be effective 11/1/10).

How Long Do I Have to Wait Before I Can Receive My Benefit?

You have a benefit waiting period of 90 days. A benefit waiting period is the length of time you must be continuously disabled before you qualify to receive any benefits.

Can the Amount of My Benefit Be Reduced?

Yes, your monthly Long Term Disability benefit may be reduced by other income you receive. For details, please refer to your plan booklet.

Is There a Limit To How Long I Can Receive My Benefit?

There is a maximum benefit period that is determined by the age that you are when you become disabled. For example, if you are less than age 61 when you become disabled, the maximum length of time that you would receive benefits would be to your normal retirement age (under the Social Security Act), but not less than 60 months.
